Referral Program

Referral bonus for existing members:

If you’re happy with the care you are receiving from us, and want someone else to experience this benefit, all you have to do is tell them about us. If that person(s) enrolls in our practice and mentions you as a referral source then you get a referral bonus. Here is how it works:

The month they enroll you get:

· Individual: $5 off your monthly membership dues the next billing cycle

· Couple: $10 off your monthly membership dues the next billing cycle

· Family: $10 off your monthly membership dues the next billing cycle

· Business owner with 5 employees or more: $20 off your monthly membership dues the next billing cycle

There are NO limits on referral bonuses; your referrals are truly the best compliment! You could literally have your membership covered in referral bonuses or I could end up owing you money - wouldn't that be nice?!

Please Note

Membership does NOT equate insurance and does not qualify for ACA tax purposes. We recommend that you carry insurance for emergency and tax purposes. The practice provides limited primary care services listed in the medical retainer agreement discussed at time of enrollment.

Membership fees will automatically be collected on a monthly basis from your credit card on file. Patients are financially responsible for all fees at time of service that are not covered in the medical retainer agreement, such as labs, imaging and medications.

This practice does not charge an enrollment fee. Membership can be canceled at any time by the patient. Members will be reimbursed a prorated amount of the 30-day fee and refunded within 30-days. There is a $300 re-enrollment fee for patients wishing to rejoin the practice as space allows in the membership.

Furthermore, if you have Medicaid or Medicare insurance we are not able to see you due to insurance regulations.
